I now use AF point expansion mode and AI Servo instead of Manual point selection and One Shot and I'm getting MUCH better results. Generally 3 out of 4 shots are razor-sharp.

I'd suggest trying to isolate the problem a bit more - AF expansion is fine, but servo mode will generally give an inferior result to 1-shot AF for subjects that aren't moving. Something just not adding up here.
