    Ahhhhgh - I'll have to think now (long day!)

    Base conversion done in ACR - saturation to zero, but a lot of playing with fill light / black clipping point and (some may be surprised to hear this), white balance.

    In Photoshop "proper" just the usual - levels mainly + HSB layer (colorise + hue to 38, saturation approx 8%) (it's a pale sepia, not greyscale).

    Bit of hand dodging / burning.

    That's about it off the top of my head - sorry that it's a bit vague
